The Virginia Worker’s Compensation Commission tries to accommodate employees who have unique situations. Employees can request an expedited (quick) hearing if they complete the form but unfortunately, this is only reserved for those cases where the employee is already under an Open Award.
The form includes the following identification information.
Employee – name, address, SSN, phone number, the spouse’s name and employment status, the names and ages of any children and dependents, available assets such as bank accounts and Social Security disability, your employer’s name, phone number and address.
The type of benefits being requested – total or partial. For what periods
The reason and support for the emergency:
- Foreclosure and foreclosure notice
- Eviction and eviction notice
- Car Repossession and repossession notice
- Medical necessity and a doctor’s statement
- Other reason
Supporting documentation must be attached, or the Commission will not consider the Application.
