What Goes Into the Virginia Worker’s Compensation Award Agreement?

    • Temporary Partial. Date of onset
    • Permanent Total. Date of onset

This payment is either marked as future compensation only or combined past and future compensation

  • Medical only. No wages. If this applies, it should be indicated.
  • Signatures required
    • Employee, employee’s attorney, employer, claims administrator

The award includes the average weekly figures and the temporary partial rates.
